For Individuals Seeking Support
Q3: Are you looking for personal support, rehabilitation, or help returning to daily life after your brain injury?
- Yes → → Self-Referral
- No → Go to Q4
Q4: Would you like to meet others who have also experienced brain injuries to share experiences and support each other?
- Yes → → Support Groups
- No → Go to “Community Wellbeing Centre” suggestion (if they need light day support or wellbeing activities)
Q4a: Have you or the person you’re supporting been affected by a brain injury and are currently involved in the criminal justice system (for example, as an offender or victim)?
- Yes → → Justice Programme
- No → Continue as above
For Family, Friends, or Carers
Q5: Are you looking to get help for someone you know who has a brain injury?
- Yes → → Make a Referral
- No → Go to Q6
Q5a: Have you or the person you’re supporting been affected by a brain injury and are currently involved in the criminal justice system (for example, as an offender or victim)?
- Yes → → Justice Programme
- No → Continue as above
Q6: Are you a professional (such as a healthcare worker, social worker, or case manager) referring a client?
- Yes → → Professional Referral
- No → Go to Q7
Q7: Would you like to learn more about how we support people day-to-day or how you can get involved?
- Yes → → Community Wellbeing Centre
- No → End or go to “Contact Us” option
For Non-Service Users (Supporters, Volunteers, or Students)
